All participants in FYL practices or events are required to complete an online Health Check questionnaire before they attend.

For the safety of all players, coaches, and spectators, we ask that you answer all questions honestly and completely. If your child is exhibiting any of the symptoms commonly attributed to COVID, please err on the side of caution, and do not attend.

The Health Check is to be completed through the TeamSnap app. To access the Health Check, simply click the event on your Home screen, and then select 'My Health Check' from the Event Details screen:
